Important tips on consultation for patients new to our office:

It it essential that we receive ALL pertinent information about your urologic condition prior to seeing you. You must be sure that all X-rays, lab tests and previous treatments are sent to us before the visit or brought with you. Without this information, our consultation will be incomplete. You MUST call us to see that we have received your records. IF NOT, you MUST call the referring physician's office just before your visit to ensure that the records have been sent and, if not, you will need to pick them up, personally, before your visit with us. Many doctor's offices, with the best intentions, do not send the records in a timely fashion. It is your responsibility to see that the records and X-rays and scans are available. Current state and federal laws require that you ONLY may obtain the records

If your insurance company requires a referral please make sure that your primary care physician has contacted your insurance company to arrange the referral before your scheduled appointment or you will be re-scheduled.  Co-pays, if required by your insurance, will be collected at the time of the visit.  We do accept VISA and Mastercard.

You should fill out the registration form and the appropriate history forms which include necessary information about your general health.  If you do not know the information in the forms, it would be appropriate to find out this information from your primary physician's office. In most cases, your primary physician's nurses or medical assistants can help you answer many of these questions about prior diseases and current medications. You do not need to speak to your primary doctor personally. 

If you have been a previous patient in our practice but have not been seen for more than two years, you will need to fill out new health forms (below)

All patients need to be sure that they bring a current list of ALL MEDICATIONS
