What
is a
referral?
A referral
is a request
from a
primary care
physician
(PCP) to
another
physician,
usually a
specialist,
requesting
consultation
on a
patient's
care.
How do I
know if I
need a
referral?
Many
insurance
companies
require a
written
referral to
see a
specialist.
The best way
to find out
if a
referral is
required is
to call your
insurance
company
prior to
your office
visit. The
number is
usually
located on
the back of
you card and
is typically
a 1-800
number.
How do I get
a referral?
Contact your
primary care
physician
(PCP). They
will be able
to generate
a referral
for you.
Insurance:
It is very
important
that every
time you see
your
physician
you bring
your
insurance
card with
you. Your ID
number or
group number
may change
yearly or
even
monthly, so
it is
essential to
bring it so
we can
always get
an updated
copy.
Co-pays:
The patient
is expected
to present
an insurance
card at each
visit. All
co-payments
and past due
balances are
due and
payable at
the time of
service. If
we have to
bill you for
co-pay,
there will
be a $5.00
fee to cover
administrative
and postage
fees.
