Has my insurance been billed?

Do you take payments over the phone?

I received a bill from Dynacare/ Seattle Radiology. Do you take care of this?

I received a statement. Is this my final bill?

Q: Has my insurance been billed?

A: When your insurance has been billed for your service, they will send you, the patient, and us, the healthcare provider, what is called ”Estimation of Benefits”. On this form, you will see the billing codes for what your doctor submitted, what your insurance allows for those codes, and what they paid for them.

If you are subjected to an annual deductible with your insurance, this information will also appear on the statement. Once we have received this information from your insurance, the remainder of the bill, if any, will be sent to you.

If your insurance pays 100% of the allowable, you will not be responsible for anything. If you have not met your deductible, or your insurance pays at less that 100%, then you will get a bill.

If we did not receive insurance information from you for whatever reason, we will, of course, not be able to bill your insurance and you will receive a bill. If you call us immediately upon receipt of your bill with your insurance information, or send us a copy of your insurance card along with your statement, we will be able to bill your insurance for you.

Q: Do you take payments over the phone?

A. Yes. Please call the billing department at 206-267-2191, and we will be happy to take your payment over the phone. Seattle Urological Associates proudly accepts Visa and MasterCard.

Q: I received a bill from Dynacare/ Seattle Radiology. Do you take care of this?

  1. No. These bills are completely separate from Seattle Urological Associates. Your physician may have ordered these tests, but the companies do their own billing.

Q: I received a statement. Is this my final bill?

A: On the statement you will see both your current charges and what is still pending on insurance. Items that are still pending with your insurance company will be marked with an asterisk (*) next to them. If you do not have any items marked with an asterisk, you do not have any items still pending with insurance, and this is a final bill for these services.
